We are looking for a talented Health Coordinator to join Baylor Scott & White Health.



Join us and demonstrate your skills and abilities by doing what you do best! In the role of Health Coordinator you will facilitate patient continuity of care with the healthcare team. This position is responsible for utilizing available resources to ensure patients receive care that is continuous, comprehensive, and coordinated across the care continuum.



The ideal candidate for the Health Coordinator role is a strong collaborator who works cooperatively with HTPN providers and personnel to carry out the goals and objectives of HTPN.
